Steps for Donating Prosthetic Equipment to the BMEL Rwanda Initiative

1. 1. Write a donation letter with your identifying information, value of items, and declaration of intent. Use the template below as a starting point. If you are donating as an individual rather than a company, then put your personal information in the spaces marked “company information”. Please list all information you have for the items, especially serial numbers.

   2. Regarding valuation, donations should be valued at the donor’s cost which would not include any mark up for profit.  A copy of cost sheet (if created), invoice (if newly purchased), or appraisal of asset is required with your donation letter.
   3. Complete IRS Form 8283
